Guidance for Completing Essential teleCalm Forms During Onboarding
As part of our onboarding process, new teleCalm subscribers may be required to complete several important forms to ensure a smooth setup and activation of services. These documents are designed to help us provide safe, reliable, and personalized phone support for seniors and their caregivers.
1. Call Management & Resident Safety Waiver
If the person receiving service (end user) will resides in a senior living community or another 24-hour supervised care setting (not a private residence), we ask that you complete the teleCalm Call Management & Resident Safety Waiver. This waiver confirms that both the subscriber and the care community understand key aspects of our call management solutions and safety practices. We recommend providing a copy to the community or care home for inclusion in the resident’s file. If the subscriber wishes to reroute 911 calls directly to a 24-hour nurse’s station within the community, there is a dedicated section in the waiver where you can initial and enter the appropriate contact number.

2. Letter of Authorization (LOA) for Number Porting
If you want to transfer (port) or keep your loved one’s current phone number when subscribing to teleCalm, you’ll need to complete the Letter of Authorization for Number Porting. This secure digital form authorizes us to work with your previous phone provider to transfer the number.

3. Priority Response Agreement
Subscribers who add the Priority Response service will receive a Priority Response Agreement. This agreement outlines the enhanced response protocols provided. It will be sent directly to your email during the onboarding process and must be completed before the service can be activated.
